Is it Possible to add Fields/Questions to the Needs Assessment Form?

Q: Regarding the assessment form, is there any ability to customize or add fields to it?

A: Yes. Each Incident is completely customizable. All Assessment forms can be changed on the fly, by request. The questions must be relevant, and we try to keep the form as short as possible to promote usability in the field.
