What client information should I include on the Needs Assessment?

Needs assessment forms are carefully crafted to minimize personal information. For example, while a team leader may want to prioritize individuals over the age of 60, the date or year of birth is immaterial; a simple checkbox indicating "Over 60" will suffice.

Crisis Cleanup should NEVER be used to store dates of birth, US social security numbers (or non-US equivalent identifiers), detailed medical information, financial data, FEMA numbers, or any other potentially sensitive information.

Crisis Cleanup is not designed to perform case management.
